Item 5.02 Departure of Directors or Principal Officers; Election of Directors; Appointment of Principal
Officers; Compensatory Arrangements of Certain Officers.
On November 13, 2015, Mark Bullivant, Senior Vice President, International, of Alphatec
Holdings, Inc. (the “Company”), announced his resignation from his position. The Company and Mr. Bullivant currently expect Mr. Bullivant to continue working for the Company through the end of November 2015 so that he may
transition his responsibilities. Mr. Bullivant’s responsibilities will be assumed by other executives of the Company.
