CFP Physicians Group Selects Allscripts for Chronic Care Management (CCM) Program
April 13 2015 - 8:01AM
CFP Physicians Group in Casselberry, Fla., selected Allscripts
(Nasdaq:MDRX) new Chronic Care Management (CCM) program to improve
care for chronically ill patient populations. The program will
enable CFP Physicians Group's participation in the Centers for
Medicare and Medicaid Services' (CMS) CCM reimbursement model
creating the potential to achieve an increase in practice revenue.
With Value-based Care Services from Allscripts Professional
Physicians Business, small physician practices can implement a CCM
solution immediately, improving care management and outcomes for
patients suffering from multiple chronic conditions. Allscripts
also helps these practices meet the requirements for reimbursement,
which includes patient outreach through care management protocols,
medication management, as well as offering monthly 20-minute
minimum of out-of-office care management services via phone or
digital coverage.
Physician practices large and small can benefit from outsourcing
the infrastructure, compliance and workflows needed to meet CMS
reimbursement requirements beyond increasing practice revenue.
Using CCM services by Allscripts, practices can experience an
increase in appointment volumes, transition of care services,
monthly medication reconciliations, and customized care planning.
These benefits can help physician practices overcome the inherent
challenges of building a CCM infrastructure without assistance.
"CFP Physicians Group is among the leading health care groups in
Central Florida because we're dedicated to providing a lifetime of
complete health care to every member of our patients' families,"
said Kelly Hidde, COO, CFP Physicians Group. "By partnering with
Allscripts for our new Chronic Care Management program, we can
advance the support of patients with two or more chronic conditions
while participating in the CMS program. In these complex cases,
enhancing our care coordination and outreach extends our ability to
effectively treat patients in the most convenient way possible, and
supports our full-service approach to health care."
"Due to time and staffing constraints, it can be difficult for
smaller practices to take advantage of this new CMS offering.
Allscripts simplifies the speed to and origination of a strong CCM
program. As a result practices can enjoy faster and larger
opportunities for additional reimbursement revenue generation,"
said Steve Lalonde, Senior Vice President and General Manager,
Professional Physicians Business, Allscripts. "In addition, the
increased frequency of patient contact can improve chronic care
management, ultimately benefiting both the practice and their
patients requiring more coordinated care. Allscripts also gives
practices the flexibility of a fully scalable program. It's a
win-win for everyone."
