Online-questionnaire provider SurveyMonkey, which has juggled leaders since the sudden death of its chief executive last year, on Wednesday named Zander Lurie CEO of the company, as he steps in for a predecessor who filled the post only since last summer.

Mr. Lurie, who previously served as interim head of SurveyMonkey as it launched a search to replace its late CEO last year, returns to the role permanently.

Mr. Lurie now replaces the man who replaced him last year, Bill Veghte. Mr. Veghte, a tech industry veteran, took the job only about five months ago. An email seeking comment from SurveyMonkey wasn't immediately returned.

The management change takes effect Jan. 16.

The 47-year-old Dave Goldberg, the former CEO of SurveyMonkey and husband of Facebook Chief Operating Officer Sheryl Sandberg, died suddenly in May.

Mr. Goldberg, a former Yahoo executive, joined SurveyMonkey in 2009 and expanded the company from 12 employees to more than 450. The company was valued at close to $2 billion as part of a $250 million fundraising round in late 2014 that was intended to help the company break into the corporate market.

A week after his death, Mr. Lurie, senior vice president of entertainment at camera maker GoPro Inc., was named interim executive chairman of SurveyMonkey.

At the time of his appointment, Mr. Lurie had served on SurveyMonkey's board since 2009, the duration of Mr. Goldberg's tenure at the company.

In August, Mr. Veghte assumed the role of CEO. He was previously Hewlett-Packard Co.'s operations chief and spent 20 years at Microsoft Corp.

Mr. Lurie will step down from his post at GoPro, which on Wednesday said it would cut about 7% of its workforce, or about 105 jobs, as it struggles with disappointing sales of its latest wearable camera.

Before GoPro, Mr. Lurie was in charge of Strategic Development at CBS Corp. and CFO of CBS Interactive. Earlier in his career, he spent six years as an investment banker in the Technology Group at J.P. Morgan.

Mr.Veghte will continue to serve on the board of directors. SurveyMonkey additionally said Wednesday that current board member David Ebersman was named chair.
