Yes I do see what you mean. I do however think that in terms of treating diabetes, both drugs show similar results in terms of weight loss and reducing Hba1c. The difference is negligible. However, Cetilistat has significantly fewer side effects. The fact that taking Xenical combined with a high fat intake causes most of the unpleasant side-effects - thus forcing patients to reduce their fat intake, is in one respect forcing patients to change their attitudes towards health & lifestyle and adopt a healthier approach, which will hopefully remain once the drug therapy has ceased & benefit them long-term. However, I would expect that if Cetilistat were to be licensed, the same NICE guidelines recommended for treatment with Xenical (weght loss of 2.5kg prior to first prescription, with continuing exercise & diet control) would apply. There are other things to consider as well, such as cost. Most PCTs run a tight ship and their budget may dictate one choice of drug over another (the cheapest) if there is very little in it (in terms of efficacy).
