Recent publication suggested that IBS affects 27% of adult USA population!
At the moment the main players are the 5-HT3 and 5-HT4 small molecule drugs:
1. alosetron 5-HT3 antagonist approved for use in female patients only with refractory diarrhoea predominant IBS. There are major safety concerns about the incidence of constipation and ischaemic colitis so this will never be a big seller.
2. tegaserod 5-HT4 agonist which has FDA approval. Not sure about safety profile.
3. renzapride combined 5-HT3 antagonist / 5-HT4 agonist. Recent pilot study showed both safety and efficacy
Also CR-1 receptor antagonists - but these are only at stage 1 trials at the moment and are not going to see clinical use in the short term.
