( 1)  This amount represents the number of shares calculated after taking into account a 1-for-5 reverse stock split which was consummated on December 28, 2015.
( 2)  12,343 shares beneficially owned by the reporting person (representing the number of shares calculated after taking into account a 1-for-5 reverse stock split which was consummated on December 28, 2015), were previously reported as directly owned, but are now being reported as indirectly owned.
