Care coordination: Meeting requirements for new CMS
Chronic Care Management program
Arbor Family Medicine (Thornton, Colorado, U.S.A.) relies on
Allscripts Professional EHR to deliver care and meet requirements
from the Centers for Medicare & Medicaid (CMS). In 2015, CMS
introduced new monthly reimbursement opportunities for physician
practices that coordinate care for patients with multiple chronic
diseases, such as asthma and diabetes.
Allscripts partner CareSync developed a
chronic care application that combines easy-to-use
technology for patients with nursing services to help providers
meet the requirements of the new CMS Chronic Care Management
reimbursement opportunity. Arbor Family Medicine was up and running

intent, we couldn't qualify to bill for it without hiring
additional staff and building an infrastructure. CareSync already
has the staff and the training to partner with us to get our
patients under control. Their work will help us hit a large number
of patient-centered medical home (PCMH) goals as well," Ann Arbor
Physician Dr. Philip J. Rosenblum said.
Efficiency: Customized appointment reminders help reduce
no-shows
Ear, Nose and Throat Associates of Texas (Frisco, Texas, U.S.A.)
is a specialty practice that uses Allscripts Professional EHR to
improve patient experience and clinical outcomes through faster
service, more effective risk management, and streamlined patient
communication and education.
The practice implemented RemindMe, an
application from Allscripts partner Talksoft, to
contact its patients with custom reminders for upcoming
appointments. Easy to set up, it took less than three hours after
the client signed with Talksoft to implement the tool proven to
reduce no-shows.
"The Talksoft set up was simple, quick and offered the
customization our office was looking for," said Mary Kay Martin,
